- A completed claims form can help show the Claims Team what you're seeking reimbursement for, but it's not required.
- If this is your first claim, the last 12 months of veterinary records (or adoption records, if applicable) are required.
- Window to file: You must file the claim within 90 days of the treatment or invoice date
- Filing Requirements: see above in “what you need” section

Family Plans
MetLife offers a unique combined coverage called Family Plans. They allow you to combine coverage for multiple pets under one umbrella policy. Here's a simple example: If you have a policy with a $250 deductible, 90% reimbursement, and $10,000 annual limit, the deductible would be combined for all your pets, the limit would be combined for all pets, and all pets would be eligible for a 90% reimbursement.Customer Support for MetLife
